Previous care and reactivating a relationship

Review ended relationships and intentionally restore active care when appropriate.

Find previous care

Professionals can open Clients → Previous clients. Families can open Your care team → Previous care team. A previous relationship is history; it does not itself grant active access to current tools.

Reactivate intentionally

Where Reactivate relationship is available, review the relationship, confirm the change, and select Reactivate. Primary reactivation must fit the professional’s capacity and may replace another active primary in the same care role. Backup reactivation still requires a valid backup assignment.

For continued postpartum care, review Care lifecycle and continued care. Follow the service’s current result if a relationship is no longer eligible.

Review continued care

If reactivation is unavailable

Contact support with account and relationship details, without medical information. Do not create a second identity or delete history to work around a relationship error.
